1. Scope of this policy

This Privacy Policy describes how Ascent Healthcare collects, uses, discloses, and protects personal information when you visit ascentpnw.com, contact us through the website, subscribe to communications, or apply for a position.

This policy does not replace any Notice of Privacy Practices that may apply to protected health information under the Health Insurance Portability and Accountability Act of 1996, commonly called HIPAA. See the Health information section below.

2. Information we collect

  • Contact information: name, email address, telephone number, company or organization, and the content of your message.

When you use the website, we and our service providers may collect technical information such as your Internet Protocol address, browser and device type, operating system, referring page, pages viewed, approximate location derived from an IP address, and dates and times of visits. We may use cookies, pixels, logs, and similar technologies for website operation, security, analytics, and performance.

5. Cookies and analytics

Cookies and similar technologies help the website function, remember preferences, understand traffic, and protect against misuse. You can use your browser settings to block or delete cookies, but some website features may not work as intended.

Where required by applicable law, we will honor recognized universal opt-out preference signals, such as Global Privacy Control, for the browser or device sending the signal. We do not respond to older “Do Not Track” signals because no uniform industry standard applies to them.

Depending on the program you join, messages may include responses to inquiries, appointment or service-related information, operational updates, recruiting communications, and, only when you separately choose to receive them, marketing or promotional messages.

  • Frequency: Message frequency varies based on your interactions with us and the program for which you opted in.
  • Rates: Message and data rates may apply.
  • Opt out: Reply STOP to any message to unsubscribe. We may send one final message confirming your opt-out.
  • Help: Reply HELP for assistance, call (503) 430-1777, or email info@ascentpnw.com.
  • Re-enrollment: After opting out, you may enroll again through the same method you originally used to opt in.
Mobile information and SMS consent are not shared for marketing. No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

We maintain records of messaging consent and opt-out requests as reasonably necessary to operate the program and meet legal and carrier requirements. Email recipients may use the unsubscribe link in a marketing email. We may still send non-marketing communications when permitted by law.

Additional messaging terms are available in our Terms and Conditions.

Applicant information may be shared with employees, contractors, advisers, and service providers involved in recruiting and hiring. We retain it for as long as reasonably necessary for recruitment, compliance, dispute resolution, and consideration for future roles where permitted by law.

8. Health information and HIPAA

Some information handled by Ascent Healthcare in connection with patient care may be protected health information governed by HIPAA or other health privacy laws.
