Terms and Conditions
The rules that govern use of the Ascent Healthcare website and participation in our communications programs.
Terms designed to be understood
Please review these terms before using our website, submitting information, or enrolling in text messages.
1. Acceptance of these terms
These Terms and Conditions govern your access to and use of ascentpnw.com, related online services operated by Ascent Healthcare, and any text messaging program offered by Ascent Healthcare. By accessing the website or enrolling in a messaging program, you agree to these terms. If you do not agree, do not use the applicable service.
Additional written terms may apply to particular services, patient relationships, employment matters, or business arrangements. If those terms conflict with these website terms, the more specific terms control for that subject.
2. Health information and medical services
The website provides general information about Ascent Healthcare and its services. Website content is not medical advice, diagnosis, or treatment and should not be used as a substitute for advice from a qualified healthcare professional.
Using this website, submitting a general form, or exchanging ordinary email or text messages does not by itself establish a provider-patient relationship. Patient care relationships are established through appropriate clinical and contractual processes.
General website forms, email, and SMS may not be secure channels for sensitive medical information. Do not send protected health information through those channels unless Ascent Healthcare specifically directs you to an approved secure method. Any applicable Notice of Privacy Practices governs protected health information under HIPAA or other health privacy laws.
3. Acceptable use
You may use the website only for lawful, personal, informational, or legitimate business purposes. You may not:
- attempt to gain unauthorized access to the website, accounts, systems, or networks;
- interfere with website operation, security, or availability;
- introduce malicious code, automated scraping tools, or unreasonable traffic;
- misrepresent your identity or submit information you are not authorized to provide;
- use the website to violate another person’s rights or any applicable law; or
- copy, modify, distribute, or commercially exploit website content except as permitted by law or with our written authorization.
We may restrict or suspend access when reasonably necessary to protect the website, users, Ascent Healthcare, or the public.
4. Website content and intellectual property
The website and its text, graphics, logos, photographs, design, software, and other content are owned by or licensed to Ascent Healthcare and are protected by applicable intellectual property laws. Ascent Healthcare grants you a limited, revocable, nonexclusive right to access and use the website for its intended purposes.
We work to keep website information useful and current, but content may contain errors, become outdated, or change without notice. Service descriptions do not guarantee availability, eligibility, coverage, clinical outcomes, or a particular result.
5. Information you submit
You are responsible for ensuring that information you submit is accurate and that you have authority to provide it. Do not submit unlawful content, confidential information belonging to another person, malicious files, or highly sensitive data that we did not request.
When you send a question, comment, suggestion, or other non-confidential feedback, you permit us to use it to respond, improve our services, and operate our business. Personal information is handled as described in our Privacy Policy.
6. Job applications
Submitting an application or résumé does not guarantee an interview, offer, or employment relationship. Ascent Healthcare may verify information, request additional information, and conduct screening as permitted by law. Employment, if offered, will be governed by separate documentation and applicable law.
Please submit only information relevant to employment. Do not include medical information, financial account details, government identification numbers, or other highly sensitive information unless specifically requested through an appropriate secure process.
7. SMS and MMS messaging terms
Program identity and enrollment
Ascent Healthcare is the sender of its SMS and MMS messaging programs. By entering your mobile number and affirmatively selecting an optional consent checkbox or otherwise completing a disclosed opt-in process, you consent to receive recurring automated or non-automated messages from Ascent Healthcare at the number provided.
Depending on the program you join, messages may include responses to inquiries, appointment or service-related information, operational updates, recruiting communications, and, only when you separately opt in, marketing or promotional messages. Consent to receive marketing messages is not a condition of purchasing goods or services or submitting a website form.
Frequency and charges
Message frequency varies based on your interactions with us and the program for which you opted in. Message and data rates may apply to messages sent to you from us and to messages you send to us. Contact your wireless provider with questions about your text or data plan.
How to opt out
You can cancel the SMS service at any time. Reply STOP to the Ascent Healthcare message you received. After you send STOP, we may send a confirmation that you have been unsubscribed. After that confirmation, you will no longer receive messages from that program unless you opt in again. If you want to join again, enroll through the same method you originally used.
How to get help
Reply HELP for assistance. You may also call (503) 430-1777 or email info@ascentpnw.com.
Delivery and carrier responsibility
Wireless carriers are not liable for delayed or undelivered messages. Delivery is subject to effective transmission by your carrier and is not guaranteed. Message services may not be available on every device or through every carrier.
Privacy
Mobile information and text messaging originator opt-in data and consent will not be sold or shared with third parties or affiliates for their marketing or promotional purposes. Service providers may process information only to support delivery, customer service, platform operation, and compliance. For more information, read our Privacy Policy.
8. Email communications
You may unsubscribe from marketing email by using the unsubscribe link provided in the message. We may continue sending non-marketing messages that are reasonably necessary to respond to a request, administer a relationship, or comply with law.
9. Third-party services and links
The website may link to websites, platforms, maps, videos, social networks, or services operated by others. Ascent Healthcare does not control and is not responsible for their content, availability, security, or privacy practices. A link does not necessarily imply endorsement.
10. Disclaimers
To the fullest extent permitted by law, the website and its content are provided on an “as is” and “as available” basis. Ascent Healthcare disclaims warranties of merchantability, fitness for a particular purpose, noninfringement, and uninterrupted or error-free operation. Some jurisdictions do not allow certain warranty limitations, so portions of this section may not apply to you.
11. Limitation of liability
To the fullest extent permitted by law, Ascent Healthcare and its officers, employees, contractors, and agents will not be liable for indirect, incidental, special, consequential, or punitive damages arising from use of, or inability to use, the website or messaging program. Nothing in these terms excludes liability that cannot lawfully be limited or excluded.
12. Changes and termination
We may revise these terms to reflect changes in our services, practices, technology, or legal requirements. The “Last updated” date identifies the latest revision. Continued use after revised terms become effective constitutes acceptance of the revised terms to the extent permitted by law.
We may modify, suspend, or discontinue all or part of the website or a messaging program at any time. Opt-out requests will continue to be honored as required.
13. Governing law
These terms are governed by the laws of the State of Oregon, without regard to conflict-of-law principles, except where federal law or another jurisdiction’s mandatory law applies.
14. Severability
If a provision of these terms is held unenforceable, the remaining provisions remain in effect, and the unenforceable provision will be interpreted as closely as legally permitted to its intended purpose.
